Subject Consent Form
I, ________________________, understand that I am being asked to participate in a study of the effectiveness of certain forms of calculus instruction involving computer technology. The study is being conducted by Lisa Denise Murphy, PhD candidate in the Department of Curriculum and Instruction, College of Education, University of Illinois at Urbana-Champaign, under the supervision of Dr. Ken Travers. If I have any questions, I may call Ms. Murphy at 333-6604, email her at firstname.lastname@example.org, call Dr. Travers at 333-7324, or email him at email@example.com. If I have questions about my rights as a research subject, I can call the Office of School-Universwity Relations at 244.0151. I have a copy of this form to keep for future reference.
I understand that I am not required to participate in the study, and that whether I participate or not will not affect my grade in Math 120 or any other course. If I agree to participate, Ms. Murphy will be able to access data from my Math 120 exams and quizzes and my mathematics placement tests and ACT score reports that are on file with the University. If I do not agree to participate, she will not have access to this information. I understand that all of the information will be kept confidential. Ms. Murphy may write about the study in terms such as "More female students than male students responded . . ." but never "Michael Jones wrote . . .". Data will never be published in any way that identifies me.
I understand that participation in this study will take about five-and-a-half hours: one-half hour for a pre-test and two evening sessions of about two-and-a-half hours each for instruction, with a post-test during the second session. At any time, I may decide not to participate any further. If I choose to drop out of the study, I may not be allowed back in. None of this will affect my grade in any course.
I understand that some of the instructional sessions may be audiotaped or videotaped. If so, the tapes will be kept confidential in the same way as the other data. At any time during the instruction, I may refuse to be taped, even if I have previously agreed to be taped. I may later be asked for an interview. If so, I am under no obligation to agree. My decisions about taping and interviews will not affect my grade in any course, or the use of my other data in the study.
Under these conditions, I agree to participate in this study.
